Personally Identifiable Information
Any participant in the discussion forum must register, provide his/her name and maintain transparency in all Public Forum postings and interactions (e.g. identify yourself as a physician). Participants may create their own site profile (name, photo, hospital/professional affiliation, certifications, areas of specialty, and medical interests) which, if created, will be displayed to Public Forum participants. Any official posting by Cook will be conspicuously identified with the Cook logo. Any other post purporting to be from Cook. Any other post claiming to be from Cook is not authorized.

Forum Usage Rules
As a Participant in the Public Forum, you agree to refrain from engaging in the following conduct:
• Harassing, threatening, embarrassing or otherwise abusing a Participant;
• Posting any vulgar, obscene or otherwise objectionable language;
• Posting any indecent, sexually explicit or adult-oriented content;
• Posting any racially or ethnically offensive language;
• Discussing, inciting or facilitating illegal activity;
• Advertising or offering other products or services for sale;
• Using the Forum to conduct a contest, sweepstakes or game;
• Interfering with an online discussion between Participants, or otherwise acting in a way that negatively affects a Participant's ability to engage in real time dialog in the Public Forum;
• Uploading, posting or transmitting any content that contains viruses, corrupt files or anything designed to interrupt, destroy or limit the functionality of any computer or telecommunications system;
• Uploading, posting or transmitting any content that infringes any copyright, patent, trademark, trade secret or other intellectual property rights held by another;
• Uploading, posting or transmitting any content that you do not have a right to submit as a result of any contractual or fiduciary obligation;
• Impersonating, or representing yourself as, another Participant or Cook employee;
• Harming minors (persons under 18) in any way whatsoever, including but not limited to, requesting personal or other information from a minor; and
• Intentionally or unintentionally violating any applicable local, state, national or international law, rule or regulation.
Cook may, from time to time, establish new rules or policies regarding Forum participation, or modify existing rules or policies. By agreeing to these Terms and Conditions, you also agree to be bound by any subsequent additions, changes or modifications to these Terms and Conditions.

Cook may modify this Forum or any part thereof at any time, or discontinue it temporarily or permanently. In the event Cook terminates this Forum, Cook may delete any content residing on the Forum site.

Cook does not endorse, nor is it responsible for, any content found on such sites or through such resources. You are fully responsible for your use of or reliance on any such content or goods or services available through these sites or resources.

Entire Agreement
These Terms and Conditions constitute the entire agreement between you and Cook regarding your use of a Public Forum, and replace any prior agreements or understandings.
